Application Form, (2) essay (3) Financial Information Form
(if applying to Academic Achievement Scholarship), to
patrons@rbhs208.org
and
(4) RB Faculty Letter of Recommendation,
(5) Outside Letter of Recommendation
mailed to Patrons' Council, RBHS, 160 Ridgewood Rd, Riverside IL 60546-2408, in
a SASE provided by the student
by Date TBA. Please consult the
Student Check List
for further information.

| Form for Faculty Reference (Word document) | Download it, fill out your portion, and give it to a faculty member
with enough lead time. The completed letters of reference should be mailed to Patrons' Council, RBHS, 160 Ridgewood Rd, Riverside IL 60546-2408, in a SASE provided by the student. The envelope should be marked with the applicant's and scholarship's name. The letters must be received by by April 15th. |
| Reference Form for Outside Contact (Word document) | Download it, fill out your portion, and either print or attach
to an email to someone outside of RB with plenty of lead time. The completed letters of reference should be mailed to Patrons' Council, RBHS, 160 Ridgewood Rd, Riverside IL 60546-2408, in a SASE provided by the student.
